Tubal ligation or vasectomy are not covered for family planning but will be covered when medical conditions of the wife justify the procedure based on one or more of the following criteria being met:
• Maternal age of 40 or greater
• Multiparity: Five pregnancies or live births
• A history of serious complications during pregnancy and/or delivery that are likely to be repeated in subsequent pregnancies
• Serious underlying medical conditions such as diabetes, lupus, renal disease or multiple sclerosis that place the life and health of the mother or fetus in severe jeopardy
•Mother has had 4 or more C sections
•Other conditions that may be reviewed are depression, habitual aborter, etc.
•Endometrial Ablation
